Ohio Benefit Bank™ Tax Clinics
Perry JFS will once again be hosting a series of FREE Tax Clinics at various branches of the Perry County District Library. These events are made possible through an OBB™ Tax Assistance Capacity Building Grant awarded to Perry JFS by the Ohio Association of Foodbanks and the generosity of the Perry County District Library.

: This PDF must be printed and filled out prior to your tax clinic appointment.
: This link can be used to file state and federal taxes for free using the OBB™ software. To create an OBB™ account, individuals must either have an e-mail account or contact Cassandra at 740-342-3551 ext. 636. Once your account has been created, you may file your taxes.
• Income Guidelines:
-Married Filing Jointly: Up to $95,000 household income
-All other filers: Up to $65,000 household income
• How to Schedule an appointment:
-Returning Clients: Call Cassandra at 740-342-3551 ext. 636.
-New Clients: Print the , fill it out, and then stop by Perry
JFS January 21st through January 24th between the hours of 8-11:30 am or 1-4:30 pm.
More dates and times for appointment scheduling will be set as needed.
• Perry JFS’ trained Ohio Benefit Bank™ counselors are NOT trained tax professionals.
They are community volunteers that have been trained to use the Ohio Benefit Bank™ software.
• There are certain situations that prevent an individual from using the Ohio Benefit Bank™
to file taxes. A partial list of these situations can be .
• Perry JFS’ trained counselors work diligently to ensure the correctness of their data entry on your behalf; however, if an
error occurs the IRS will reject the submission and send you a letter with the reason why. Once you receive the information,
contact Cassandra at 740-342-3551 ext. 636 so that the mistake can be corrected and your taxes resubmitted.
There is no penalty for this kind of error!
• If you do not have all of your forms, Perry JFS’ trained counselors can not file your taxes.
If you receive forms after you have filed, you will need to contact a CPA to file an addendum on your behalf.
